Ever since I've been in coding, there's been issues with the 25 modifier. But Cigna is making an overly burdensome policy of requesting ALL documentation for 99212-99215 with a 25 modifier. Fax or email, no indication of a nice HIPAA secure portal. More work for providers and reviewers.
